결과요약 및 제언
본 연구는 정맥주사 관리에 대한 간호사들의 인식과 수행에 관한 실태를 파악하고 수행 저
해요인을 규명하여 올바른 정맥주사 관리방안을 마련하는 데 기초자료를 제공하고저 시도된
서술적 조사연구로서 부산시내 소재 5개 종합병원 간호사 420명을 대상으로 하였다.
연구도구는 Simmons 등(1982)의 정맥내 감염예방지침, Stanley(199)의 「Ⅳ Therapy]와
Kurdi (1996) 의 「Modern Intravenous Therapy Procedures] 및 임상간호사회(1992), 부산
시내 3개 종합병원의 간호 실무지침서(1996) 등의 문헌고찰을 통해 정맥주사 관리에 관련된
문항을 본 연구자가 수정, 보완하여 작성한 설문지로서 주입전, 주입직전, 주사바늘 삽입시,
주입중, 주입후 관리의 5개 영역의 총 68문항으로 구성되었으며, 신뢰도는 인식이
Cronbach's α=0.93이었고 수행이 Cronbach's α=0.87이었다.
수집된 자료는 SPSS/PC+ 프로그램을 이용하여 실수와 백분율, 평균, 표준편
차, paired t-test와 t-test 및 ANOVA로 분석하였다.
본 연구결과를 요약하면 다음과 같다.
1) 정맥주사 관리에 대한 인식은 평균 4.59±0.33으로 수행의 평균 4.46±0.29보다 높았으며
5개 영역중 주입전(t=8.58, p<001), 주입직전(t=6.76, p<.001), 주입중 관리(t=4.82, P<.001)의
3개 영역에서 수행이 인식에 비해 낮은 것으로 나타났으며 이는 통계적으로 유의하였다
(t=5.86, p<.001).
2) 청맥주사 관리 문항의 평균 수행점수가 영역별 평균 수행점수보다 낮았던 문항은 주입
전 영역에서는 9문항으로서 약물의 확인, 주입전 손씻기, 수액 고무마개의 소독, 항암화학요
법전 검사결과 확인과 일회용 비닐장갑 등의 착용에 대한 것이었고 주입직전 영역에서는 6
문항으로 손씻기, 투약 전 환자 교육, 주사부위의 선정과 소독방법에 대한 내용과, 주사바늘
삽입시 영역에서는 4문항으로 주사삽입 시도 실패시 다른 간호사가 시행, 반창고 사용, 지지
대의 사용에 대한 내용, 주입중 영역에서는 13문항으로 지속적 정맥주입시 주사삽입부위의
기록, 조직 침윤시 관리, 수액세트 교환, TPN 주입시 기록사항, 주입용액의 사용기한,
3-way 사용시 소독에 대한 내용이었으며 주입후 영역에서는 3문항으로서 투약 후 환자상태
의 관찰과 기록, 주사바늘 제거시 바늘길이 확인에 대한 내용이었다.
3) 정맥주사 관리의 수행 저해요인으로 모든 문항에서 시간부족 때문이라고 응답하였고 그
중 특히 손씻기가 시간부족으로 수행이 잘 안 된다고 응답하였으며, 항암화학요법시 일회용
비닐장갑과 마스크, 보안경의 착용은 물품부족으로, 약물의 적합성 확인은 방법을 몰라서,
지속적 정맥주입시 주사 삽입부위의 기록에서는 불이익이 없어서, 항암화학요법시 일회용
비닐장갑과 마스크, 보안경의 착용, 중심 정맥관의 드레싱 교환, TPN 주입시 혈당 측정에서
는 의사의 업무이므로 수행하지 않는 것으로 응답하였다.
이상의 연구결과를 볼 때 정맥주사 관리에 대한 간호사의 인식과 수행이 모두 높았으나 인
식에 비해 수행이 낮은 것은 약물의 확인과 투약 전 환자교육, 손씻기, 항암화학요법시 일회
용 비닐장갑과 마스크, 보안경 착용, 주사부위의 선정과 소독, 지속적 정맥주입시 주사 삽입
부위의 기록, 조직 침윤시 관리, 수액세트 교환 등이었다 또한 수행의 저해요인으로 시간부
족이 가장 많았고 그 외에도 물품 부족, 수행방법을 모르거나 수행하지 않아도 불이익이 없
음, 의사의 업무 등으로 나타났다.
따라서 간호사의 정맥주사 관리에 대한 인식과 수행을 높일 수 있도록 약물에 대한 지식,
감염예방, 환자의 안위와 기록의 중요성을 강조하는 실질적인 교육 프로그램의 시행과 함께
적정 간호 인력의 배치, 싱크대 설치와 종이타월의 보급, 손보호를 위한 대책마련 및 정맥주
사 관리시 필요한 물품의 적정량 제공을 위한 제도적, 재정적 뒷받침이 필요하다고 본다.
이상의 결론을 토대로 다음과 같이 제언하고자 한다.
1) 신규 간호사 교육이나 보수교육시 정맥주사 관리에 대한 인식에 비해 수행이 낮은 문항
을 중심으로 하여 교육내용에 수행을 높이는 방안이 포함되어야 한다.
2) 정맥주사 관리에 대한 간호사의 인식수준까지 수행을 끌어올릴 수 있도록 물품공급과
계속적인 관리, 정책의 수립과 제도의 마련이 요구된다.

The purposes of this study were to identify nurse's recognition and performance and to
represent the factors of hindrance in the Ⅳ therapy management. The subjects were 420
nurses who worked at five general hospitals in Pusan.
Tha data were collected using a questionnaire and the period of data collection was
from January 1 to January 31, 1998.
The instrument for this study was made by author oneself on the basis of guidelines
Simmons et al', CDC' Stanley' and Kurdi' guideline, Clinical Nurse's Association' that
consist of 68 items for 5 fields : pre-injection, just before-injection needle-Injection,
during injection, post-injection field. Cronbach Alpha coefficient of recognition and
performance in the Ⅳ therapy management was .93 and .87
The datas were analized by a SPSS program using frequency, percent, paired t-test,
t-test and oneway ANOVA.
The results obtained were as follows :
1. The mean score of recognition in N therapy management was significantly higher
than that of performance (t=5.86, p<.001).
2. The items of lower than mean score of each fields in performance were the
identification of drugs, hands washing, patient teaching about medication, disinfectional
methods of the injection site and the rubber stopper in bottle, the use of disposable
gloves, mask and eye goggles at the chemotherapy preparation, use of tape and
armboard, changing the Ⅳ tubing, labeling the dressing over the injection site,
observation and recordings of patient's condition after medication and confirmation of
the needle length at the needle removal.
3. The factors of hindrance in Ⅳ therapy were 'having no time', 'insufficiency of
goods', 'unknowing of methods ', 'no disadvantage', and 'factors of doctor's doing'
The most important factor was 'have no time', especially item of hands washing. The
other factors of hindrance showed high frequency in the following items ; 'insufficiency
of goods' in the use of disposable gloves, mask and eye gogg1es at the chemotherapy
preparation, 'unknowing of methods' in the certification of drugs compatibility, 'no
disadvantage' in the labeling the dressing over the injection site, and 'factors of
doctor's doing' in the changing the subclavian catheter dressing and checking the
glucose level during the TPN infusion.
In conclusion, there is necessity of educational program which can improve the nurse's
knowledge of drugs, disinfection methods, comfort of patient and recordings in Ⅳ
therapy management and alternative plan which are political and financial aids such as
setting up the sink, giving of paper towels and necessary goods in the Ⅳ therapy for
reducing the factors of hindrance for Ⅳ therapy management.
